Crucell Vaccines requires a RETURN AUTHORIZATION NUMBER in order to return expired product. A completed RA Request Form must be submitted prior to returning product. Please contact Customer Service (800) 533-5899 for details.
TERMS OF DIRECT RETURN POLICY:
1. Credit will be issued based upon current selling price, or the actual invoice price, whichever is lower.
2. No cash refunds will be made.
3. All direct returns will be subject to a 15% handling charge.
4. All outdated products must be shipped prepaid to: RA number must appear on mailing label.

5. Notification of product error, damage or loss during transit must be made within ninety-six (96)
hours. Damage must be inspected by a carrier before Crucell Vaccines will arrange for return or
destruction.
6. Returns purchased from authorized wholesalers must include Wholesaler's name and address,
Customer DEA Number, Wholesaler's customer number, & customer's group purchasing affiliations.
7. Returns must be made within two (2) business weeks upon receipt of RA number. Credit will not be
issued for returns after the 2 week period.
8. The following are NON-RETURNABLE classifications:
A) Opened or partial packages
B) Products sold on a non-returnable basis.
C) Products expired more than four (4) months.
D) Products returned by anyone other than the original purchaser.
E) Products damaged due to improper handling or storage
F) Products not in original container
TERMS OF RETURN POLICY VIA PROCESSING FACILITY:
1. Credit will be issued based upon current selling price, or the actual invoice price, whichever is lower.
2. Returns purchased from authorized wholesalers will be credited to the wholesaler.
3. All returns via processing facilities will be subject to 15% handling charge.
4. Crucell Vaccines shall maintain final discretion on acceptance or rejection of returns listed on
processing facility's documentation.
5. The following are NON-RETURNABLE classifications:
A) Incomplete/inaccurate lot numbers, NDC #'s, and expiration dates.
B) Products expired more than four (4) months.
C) Partial units.
D) Recalled products
